Most conversations about burnout happen in the language of the mind. Exhaustion. Cynicism. Reduced efficacy. These are the three dimensions identified in the Maslach Burnout Inventory, and they are real, but they are not the whole picture. By the time burnout is visible in those psychological categories, the body has usually been speaking for months.
The physical signature of burnout is something I spend considerable time on in High-Functioning Burnout, because it is the dimension most consistently missed, especially by high performers who have learned to read their body's signals as noise to be managed rather than data to be understood.
What Chronic Stress Does to the Body
Chronic stress, the sustained activation of the sympathetic nervous system over weeks, months, or years without adequate recovery, produces measurable physiological changes. These are not metaphors. They are documented biological processes. Cortisol, the primary stress hormone, plays a crucial regulatory role in the short term. But in chronic elevation it damages immune function, disrupts sleep architecture, increases inflammatory markers, and contributes to the kind of low-grade physical unwellness that high-functioning people tend to dismiss as just the way things are now.
A 2024 cross-sectional study of 1,540 emergency nurses found a positive correlation between burnout scores and somatic symptom severity, the higher the burnout, the more pronounced the physical complaints. That correlation is not specific to nursing. It is a human biological pattern. As Bessel van der Kolk's widely cited formulation puts it: the body keeps the score. The physical patterns of chronic stress become encoded in muscles, breathing, posture, and movement. They persist even when the stressor is removed.

The Physical Signs People Ignore
Persistent tension in the jaw, neck, or shoulders is not stress. It is the body bracing for threat that has not materialised, holding the posture of readiness because the nervous system has not received reliable signals that the emergency is over.
Disrupted sleep despite exhaustion, the inability to fall asleep easily, the waking at 3am with a racing mind, is the sympathetic nervous system maintaining vigilance. Frequent minor illness is the immune system rationing resources. Digestive disruption, skin flares, hormonal irregularity, these are all documented somatic expressions of chronic stress. In High-Functioning Burnout, I call these the early warning system. Most high performers override them. That override is itself part of the condition.
Why Rest Alone Doesn't Work
This is the insight that most recovery frameworks miss. When the nervous system has been in chronic sympathetic activation for an extended period, it loses the capacity to down-regulate efficiently. Taking a week off and returning in the same condition, or worse, is not a failure of commitment to recovery. It is a physiological fact.
The recovery work, real recovery, is active. It involves deliberately using body-based practices to retrain the nervous system's capacity for down-regulation: breath work that activates the parasympathetic branch, movement that discharges accumulated sympathetic activation, physical safety cues that allow the threat-detection system to stand down. Somatic Experiencing research, including a 2017 randomised controlled trial by Brom and colleagues, demonstrates the effectiveness of body-based approaches for conditions that directly overlap with burnout presentation.
Redesigning the Conditions
Recovery from burnout requires two things, not one. Restoring the nervous system's capacity to regulate. And examining the structural conditions that depleted it. Without both, recovery reloads the same system. This is the Somatic Collapse Loop I describe in High-Functioning Burnout, and the reason the pattern repeats. The body's insistence is not weakness. It is information. The question worth sitting with is: what is my body trying to tell me that my mind has been too busy to hear?
